I agree with you (13/3 23:37 +0100 and 23:41 +0100) that 'Why?' may have
started the 4th level. As I see it 'Why?' (asking explanation/motivation)
implies valuing reflecting/understanding/individual choice over just doing
what everyone does and what must therefore be the best to do (even it the
first person asking 'Why?' couldn't possibly have explicitly stated these
values).

I agree the MoQ is just another intellectual pattern of values and is not in
itself a start of a 5th level. A MoQ may help though to 'explore what might
be the highest values accessible to people' (beyond 'truth') IF it is not
used as 'an inadequate evolutionary explanation that really explains
nothing' (quoting from John B.'s 16/3 2:13 +1000 post in a way contrary to
his intentions). I see the idea of DQ (distinguished from sq) that BOTH
founds static patterns of values AND lures them into migrating towards it as
more than an evolutionary explanation. 'Evolution' is no more than a
metaphor in this context.
